Unison Health and Community Services provides free, confidential and non-judgmental services to people of all ages, cultures and backgrounds. Unison aims to work together to deliver accessible and high quality health and community services that are integrated, respond to needs, and build on strengths and change. The Health service is a client-centered, multi-disciplinary approach to primary health care. This means that the clients are partners in the planning and decision making about their treatment and/or care. A team of doctors, nurse practitioners, nurses, social workers, case coordinators, dietitians and chiropodists work closely together. They provide the best information and guidance to clients and support them in the decisions they make about their health care. The team of practitioners encourages clients to make use of the health promotion programs both at Unison and in the community. The primary health care approach also aims to understand the factors outside the health system that can influence the health of the individual and the community as a whole such as poverty, housing issues, food insecurity and uninsured clients.
